Before you get started
This form will provide you with useful information to help you to remain independent. Where you need further support or you are already receiving services from Social care you will be able to make a referral.

Urgent Need

  • If you are at immediate risk from harm please call the Police
  • If you're concerned about someone's health in general, contact a healthcare professional such as your GP, NHS 111 or for emergency situations 999.
  • If you have a social care emergency and need urgent assistance please contact us on 01582 547 659 between 9am and 5pm Monday to Friday. If it is outside of these hours please call 0300 300 8123.
  • If you have found that you are homeless please present at your local council
